National Bankruptcy Forms include the Voluntary Petition for Individuals that have been approved by the Judicial Conference of the United States and are required to be used in all bankruptcy courts. Most forms are drafted and approved by the Administrative Office of the United States Courts. The use of these forms is recommended.
